creating evaluation form...

I am creating an evaluation form for my GoToMeeting class. I did this previously and remember that there were options for the type of answers allowed - like multiple choice, comment, text, etc. I can't seem to find that option anywhere this time around. It's only giving me multiple choice option. What am I missing?

It was the browser. Strange....because I used Firefox previously, but this time Firefox didn't work. Must be different versions on my two computers.

If you don't have the drop-down menu to select different question formats, have you tested in an alternate web browser?
